Adoption Questionnaire

The adoption process includes filling out this questionnaire and signing an Adoption Agreement. Although we do not require it, we do prefer that your new family member be kept inside. We do require a fenced area for outside play. These requirements help us to find the the best home possible for our dogs, ensuring that they do not return to the streets or end up in the shelters, at risk for euthanasia. We appreciate your understanding. First and foremost, we have the dogs' best interest in mind.

We check Vet and personal references and conduct home visits.

Out of state adoptions are welcomed provided the adopters cover transportation costs. We can also attempt to find space on a volunteer transport for out of town adoptions.

Adoption Fees
Our adoption donation fee is $125 and up, depending on the medical expenses incurred by Dogs2ndChance.

Below is a rough example of what our costs average per dog. Please keep in mind that we receive a deeply discounted rescue rate with our vet, whose prices are normally significantly higher. Also prices listed are for small dogs and on most services prices increase with weight.

  • $10+ Leashes, collars, tags, etc.

  • $75+ Initial Vet Visit - includes 7 in 1, bordatella, rabies vaccinations, heartworm test, ehrlichia/lyme test and fecal

  • $70+ Spay/Neuter

  • $15+ per month - Heartworm and Flea/Tick prevention

  • $40+ Treatment with Drontal or Panacur (to deworm, and eliminate coccidia or giardia if present. These are common southern shelter problems.)

  • $10+ per day Boarding (when necessary until foster space opens up)

  • $200+ Heartworm treatment with medications (many of our dogs come into rescue HW+, when they test negative it is a gift)

  • $ All other necessary veterinary treatment

So the average costs incurred for a 'healthy' dog, that only needs vaccinations, spay/neuter and HW/Flea preventative is approximately $170. As you can see from the prices above, many cost MUCH more.

You should also keep in mind that the average cost to care for a large breed pet per year is approximately $1500. This is for food, annual check-ups and things like that. This amount could easily double or even triple should your dog become ill and need extensive tests, surgery or other treatments. So please carefully consider if you are able and/or willing to invest this into your pet. If not, PLEASE wait to adopt!

This questionnaire must be COMPLETED IN FULL.

Our Adoption Process

Our adoption process includes signing an Adoption Agreement, in addition to completing this questionnaire. We check Vet references and conduct home visits. Although we do not require [but prefer] the dogs to be kept inside, we do require a fenced area for outside play. These requirements help us to find the the best home possible for our dogs, making sure they do not return to the streets or shelters.

We appreciate your understanding that our primary concern is what is best for our adoptable dogs.

The adoption donation amount is $125.00 and up, which is used to cover a portion of veterinary expenses for the dogs in our care.  Frequently medical expenses amount to more than this for each animal.  Extra donations are greatly appreciated!

We welcome out of state adoptions, providing that the adopters cover transportation costs.  We may also be able to find space on a volunteer transport for out of state adoptions, assuming all previously stated contractual requirements have been met including a home visit and check of vet references.

All the above statements are true to the best of my knowledge. I have read and understand all portions of this document.
