Pet Adoption Application

Click here for a list of pets at this shelter

Save A Pet is a small group of dedicated people helping to place abandoned, sick and homeless pets into new loving forever homes. We use this application to get a feel for you and to also help us know if the dog/cat you are interested in is a good fit for you and your lifestyle.  The more you elaborate the more we get a sense of you as well as how serious you are about this application.  After we receive the application vet references are checked and then we contact you.  We require the application first for many reasons.  First of all there are time constraints, we all work and have families and phone work is costly from a time perspective, secondly it gives us a base to start from when trying to organize our paperwork.  -

Why would you like to adopt a pet?

What do you think are the most important responsibilities in owning a pet?

For whom are you adopting the pet?

Have you ever owned a pet before?

Yes No

Please describe those pets that are currently with you (type, name of pet, age, sex, altered status, declawed)

Please describe those pets that are no longer with you (type, age, sex, altered status, declawed) If you can please let us know how long ago these pets expired.  Sometimes when we call your vet they have no record of the animals with you as it was many years ago and those records are files in a different area.  This can make it appear that your information is incorrect.

What happened to the pets that are no longer with you?

Please provide all the name(s) and phone number(s) of the veterinarian(s) for your current and/or former pets. We do call your vet to check your reference.  We also need to know the name your account is under at your vets office.  If it your spouse, your maiden name or some other name.  Lately many vets will not give out vet information without approval from the owner. You may want to call your vet and let them know that we will be calling otherwise it takes significantly longer to process your application and if the animal is popular it will cost you the adoption.
