Questionnaire

Anyone wishing to adopt an animal must fill out our adoption questionnaire. This helps us to place our animals in the best home possible. Just click on the link below, fill out the form, and submit it. It will be emailed automatically to our shelters manager. If you are coming from far away, make sure that you get a response from us, this will help avoid any unnecessary trips. (note: the I.D number requested on the form is not the Petfinder I.D. but our four digit one) After we have emailed you back about your application you must call the shelter, at your expense, Monday to Friday, 1-5 or Saturday, 12-4 to have a conversation with either Norma Savage , Manager or Regina Caswell, Assistant Manager. If you reach the voicemail or the answering machine please call back.

Please Note: If  you have not previously adopted from us, there is a 24-hour wait before your application will be approved. For all adoptions we require proof of homeownership or an animal consent statement from your landlord with landlord's name address and phone number.
