NCSHAR (North Carolina Siberian Husky Animal Rescue) thanks you for your interest in our foster dogs. We appreciate you taking the time to adopt a rescued dog rather than purchasing one. Adopting may take a little longer, but you get the satisfaction of knowing you have saved a life! The dog you adopt knows it too. Rescue dogs are grateful for anything you give or do for them. We would appreciate the answers to the following questions, so we may best select the right dog for you. All information is confidential. Submission of an application does not mean you will receive a dog.

INCOMPLETE APPLICATION MAY BE REASON FOR REFUSAL OF ADOPTION.

I understand completing an application does not mean I will be able to adopt a dog from NCSHAR.


I understand the foster dog has already suffered the loss of one family. I will support NCSHAR's efforts in facilitating the next home will be a permanent one by allowing NCSHAR to -

1.Interview me and talk to my personal references.

2.Discuss my present and previous pets and their care with my Vet.

3.Make a scheduled visit to my home to inspect the fences, and areas where the dog will be kept.


I understand a dog adopted through NCSHAR must be kept within a fenced yard, on a leash, appropriate tie out, or in my home at all times. The dog may never be chained or staked outdoors on a permanent basis.


I understand there will be an adoption fee, which will be used to partially offset the medical expenses incurred by NCSHAR.


I understand all rescued dogs must be spayed or neutered - a rescue dog will NEVER be bred.


I am able to afford regular (at least annual) Vet care for my dog, including heartworm preventive and vaccines, and emergency care if needed.


YOUR ELECTRONIC SIGNATURE AUTHORIZES NCSHAR TO CONTACT YOUR REFERENCES AND VERIFY THE INFORMATION PROVIDED IN YOUR APPLICATION.

 Please realize incomplete or inaccurate information could be reason for refusal of adoption.

SHOULD YOUR APPLICATION BE APPROVED, THIS DOCUMENT WILL BECOME PART OF YOUR ADOPTION CONTRACT. MISREPRESENTATIONS IN THIS APPLICATION DISCOVERED AFTER ADOPTION OF A DOG FROM NCSHAR COULD RESULT IN THE IMMEDIATE FORFEITURE OF THE DOG WITH NO COMPENSATION DUE.
