I hereby acknowledge receiving from Kootenai Pets for Life, Inc. the animal described on the following page and hereby agree:

• To keep this dog/cat in my personal possession, provide proper & sufficient food, water, shelter, grooming & humane treatment at all times.
• I will return the animal to Kootenai Pets for Life within one month for a refund of adoption fees if the adoption is not compatible.
• Not to sell, trade, transfer ownership, abandon or dispose of this dog/cat in any way, but to notify KPFL if I must relinquish custody of this dog/cat. This includes release to family members.
• To keep this dog/cat as my household pet & companion.
• To insure that when outside & unattended, this dog is in a secure fenced yard or kennel run with adequate shelter from the elements. To exercise this dog within a fenced yard or kennel run or by walking this dog on a leash, & to never allow this dog to run loose without adequate adult supervision. To never chain or tie this dog without being in attendance.
• This animal shall never be used for medical or experimental purposes.
• KPFL will call you to see how dog/cat is adjusting and how you are adjusting.
• KPFL makes no guarantees about the health of this animal.
• KPFL will not be held responsible for inquiries or destruction of property caused by the animal after its adoption.
• I understand that it is my responsibility to see & evaluate the dog/cat for myself before agreeing to adopt it.
• It is mandatory that all pets not altered at the time of adoption due to age be spayed/neutered by six months of age. Failure of the owner to follow through will result in surrender of the pet to KPFL. Proof of spaying or neutering will be provided in writing to KPFL within ten days of surgery. I will not allow the dog to breed or to be bred under any circumstances.
• To procure veterinary care at once if this dog becomes sick or injured and to keep current all vaccinations.
• I am in full agreement with these terms of adoption.

All expenses incurred (vaccinations, spay/neuter, etc.) after the adoption are the responsibility of the new owner.

I agree to all of the above conditions and am responsible for adherence to this agreement. (We suggest you make a copy of the agreement for your records).

Suggested donations are $60.00 dogs and $25 cats. Each animal is either spayed or neutered and has had shots before being released for adoption. Our costs run between $80.00 and $100.00 for each pet we place, therefore the suggested donations covers just a portion of our expenses. It also includes a one year membership in Kootenai Pets for Life.
