1. If you are interested in a specific cat listed on our site, please fill in the animals name. You may also describe the type of cat you are looking for so we may keep an eye out for you.

2. Where will your cat be kept during the day, at night, and when left alone. Do you plan on letting your cat go outside?

3. Please explain your living situation. Do you own or rent? Do you live in an apartment, house, or mobile home? How many years have you lived there? If you rent, please provide the landlord's name and telephone number.

4. Please list all people living in your home, including ages.

5. Do you own any other pets? Please list ages and breeds, and if they are spayed or neutered (if not, please explain why).

6. If you own other pets or have in the past, please list your vetrinarian's name and phone number.

7. Please explain who will be home with the cat in the days and evenings. How long will it be left alone?
