1. If you are interested in a specific animal listed on our site, please fill in the animals name.

2. Where will your dog be housed? If outside, what kind of shelter will your dog have? Please explain how you plan on letting your out to use the bathroom (how often, fenced in yard, on a leash etc...)

3. Please explain your living situation. Do you own or rent? Do you live in an apartment, house, or mobile home? How many years have you lived there? If you rent, please provide the landlord's name and telephone number.

4. How do you plan on exercising your dog? (Fenced in yard, park nearby, walks every night, etc...)

5. Please list all people living in your home, including ages.

6. Do you own any other pets? Please list ages and breeds, and if they are spayed or neutered (if not, please explain why).

7. If you own other pets or have in the past, please list your vetrinarian's name and phone number.

8. Please explain who will be home with the dog in the days and evenings. How long will your dog be left alone?
