 hereby gift, give, grant and convey unto ALL BREED RESCUE and REFERRAL (ABR) or its representatives, successors or assigns, full and complete title, interest and/or ownership in and to my pet.

I fully understand that by executing this document and delivering the above described pet to ABR, that I hereby relinquish all legal and equitable rights of ownership of the above described pet to ABR to have and to hold forever.

I understand that this decision cannot be rescinded.

I represent that I am the lawful owner of and have good title to and the right and power to relinquish said pet and that said pet is free and clear from claims of ownership or other encumbrance of any kind or nature whatsoever by any person or entity and that I will warrant and defend unencumbered ownership rights to said pet hereby relinquished again the claims and demands of all persons.

By signing this document, I attest this pet has never, to the best of my knowledge, bitten and broken the skin of anyone except as described below:
