If you rent, will your landlord confirm that this pet will be allowed?
Landlord's name and phone number:

Does anyone in the household have allergies to pets?

6)How many hours would the pet be alone (with no pet or human company) during the day?

QUESTIONS 7 & 8 IF FOR A CAT:

We request a minimum donation of $125 per dog, $125 per altered feline (adult or kitten), with an additional $25 for combo tested cats (for feline leukemia, FIV).
