Pet Castle, Inc. Animal Rescue
We
make every attempt to match prospective adopters with a pet that best fits the
adopter's home and lifestyle. Submitting this application does not
obligate you to adopt, nor does it guarantee your application will be
approved.
Please
keep in mind, it is common to receive more than one application/request on the
same animal.
Submitting
this application in no way guarantees your adoption of this animal from The Pet
Castle, Inc. Animal Rescue (TPC).
Submitting
this application attests that the information you have provided is accurate and
true and also give your current and past veterinarian(s) permission to provide
any and all information requested by TPC in regard to medical and preventative
care provided any current and/or past pets they have on record.

E - Terms of the Agreement
By
submitting this agreement, you agree to the following:
o I
acknowledge full responsibility immediately upon adoption of the animal
described above and will not hold The Pet Castle, Inc. Animal Rescue responsible
for any actions or illness of the said animal.
o I
agree to contact The Pet Castle, Inc. Animal Rescue if my pet becomes missing or
passes away.
o I
agree to care for my pet in a responsible manner. I agree that my new dog
shall have food, water and adequate shetler at all times and/or that my new cat
shall remain indoors at all times.
o I
agree that if my desired pet is not fixed at the time of the aoption, I am
required
to have the animal spayed/neutered,
as soon as possible (when age permits). I acknowledge that only in rare
cased, due to medical conditions approved by a vet, will this not be
required.
o I
agree to vaccinate my pet as required, including rabies vaccines.
o According
to my local laws, I agree to license my pet with the county in which I reside.
I understand that purchasing a tage for my new pet is my
responsibility.
o I
agree I will not surrender this pet to an rescue organization, county shelter
(or any other animal related group) without first contacting The Pet Castle,
Inc. Animal Rescue for written consent.
o I
agree that I will not, under any circumstances, sell or give this pet away
without written consent from The Pet Castle, Inc. Animal Rescue.
o I
agree that if circumstances arise whereby I can no longer maintain my
guardianship of this animal, I will contact The Pet Castle, Inc. Animal
Rescue.
o I
understand that The Pet Castle, Inc. Animal Rescue reserves the right to retain
my adoption fee is this pet is returned for any reason.
o In
the event no foster home is available when I return my pet, I agree to house or
safely board my pet until such time as the space becomes available. I also
understand that boarding fees are my responsibility.
o I
agree that if my dog becomes missing and is picked up by Animal Control I will
"bail" my pet out when necessary. I also agree that if The Pet
Castle, Inc. Animal Resuce "bails" my pet out, I will reimburse The Pet Castle,
Inc. Animal Rescue for those fees before my pet will be returned to
me.
o I
agree that if I return this pet unannounced for any reason, or abandon this pet
anywhere, a $50.00 surrender fee will be charged to care for my pet. I
also understand that pound fees will be charged in addition to the surrender fee
referenced above in the event my pet is picked up by Animal Control and The Pet
Castle, Inc. Animal Rescue chooses to "bail out" my pet. (legal action may
or may not be taken)
o I
agree that if so any reason the adoption fee is not received (i.e. bounced
check, etc.) The Pet Castle, Inc. Animal Rescue reserves the right to remove the
animal from my home. I also agree that I will reimburse The Pet Castle,
Inc. Animal Rescue any fees associated with writing a bad check.
o In
the event that The Pet Castle, Inc. Animal Rescue must bring legal action
against me, I agree to pay an reason legal fees associated with said action.
I also agree that any legal action against me will be in the Kenton
County, KY Court System regardless of where I am living at the time of said
action.
o I
understand and agree that The Pet Castle, Inc. Animal Rescue can check up on my
pet by telephone, email or visitation. I also agree The Pet Castle, Inc.
Animal Rescue reserves the right to remove my pet if it is found that they are
not being properly cared for and/or if the terms of this agreement are not being
met.
o I
also attest that everything submitting in this application/agreement is accurate
to the best of my ability. I also attest that I am over the age of 18 and
can legally enter into an agreement.
If
this application/adoption is approved, you must have a photo ID available at the
time of the adoption. Also if you current address is not on this photo ID,
you must provide proof of your current address that is listed in this
application. (i.e. copy of phone bill, electric bill, etc.)
The
Pet Castle, Inc. Animal Rescue suggests that before submitting this agreement,
you should select print in your browser so that you have a copy for
your records.
The
Pet Castle, Inc. Animal Rescue has vet checked, de-wormed and given at least the
first set of shots for this pet. Puppies and kittens require a series of 3
sets of vaccines. The Pet Castle, Inc. Animal Rescue advises that you have
a vet check performed by your own veterinarian within two weeks of the date of
the adoption.
I acknowledge that I have read and agree to the above terms and agreements before submitting this application.
