Foster homes are required to provide heartworm preventative and Frontline or Advantage and must be administered on a monthly basis, unless against veterinary advice (I.E. if dog has heartworms). 

By completing this application and signing below, I certify that I am at least 21 years of age and will:

1. Assume all responsibility for the care and well being for the animal that I am fostering through Underdog Rescue of Florida.

2. Provide heartworm preventative and Frontline or Advantage and administer on a monthly basis, unless against veterinary advice (I.E. if dog has heartworms).

3.  Utilize approved veterinarians of Underdog Rescue of Florida and notify a representative when veterinary attention is needed.  Exceptions will be made for medical emergencies. 

4. NOT release the foster dog to another person without express permission from a representative of Underdog Rescue of Florida.

5. Exercise extreme caution and care in acclimating the dog into its foster home and inhabitants and to hold Underdog Rescue of Florida and its representatives free and harmless from any loss, damages, injury or death to person, animal or property caused by the dog while in foster care.
