Adopting person(s) agrees that rescue dog will be a house pet and will receive all care and attention necessary to ensure his/her well being. Adopting person(s) will provide adequate food, water, shelter, exercise, veterinary care and yearly inoculations, heartworm test and monthly preventative, and any other treatment as recommended by attending veterinarian and license said dog in accordance with state and local laws.

Adopting person(s) will not permit dog to run free or off leash out of immediate control. When outside, the dog will never be staked out or chained.

Adopting person(s) will provide indoor sleeping quarters.

Adopting person(s) will not transport the dog in the bed of an open pick-up truck

Adopting person(s) agrees that at all time dog will wear a buckle collar with an I.D. and Rabies tag.

Adopting person(s) will notify NC Saint Bernard Rescue within 1 week of change of address or telephone number.

Adopting person(s) will keep the rescue dog for as long as he/she shall live and will NOT transfer ownership to any other person or organization (Including an animal shelter) for ANY reason.

If custody (Whether permanent or temporary) of the rescue dog must be relinquished, adopting person(s) agree to notify the NC Saint Bernard Rescue immediately to arrange for his/her return to the NC Saint Bernard Rescue at the dog owner(s) expense.

The adopting person(s) takes the rescue dog with all defects either observable or unobservable, and assumes full risk for the dog from the date of signing this agreement. Every care is given to the physical condition and temperament evaluation of each dog that is accepted by the NC Saint Bernard Rescue, and every possible effort is made to provide a rescue dog that is compatible with the adopter(s) home, no warranty is made to the physical condition or temperament of the adopted rescue dog. The following characteristics of this dog's temperament has been observed:

As of the date of this agreement, the adopting person(s) assumes responsibility fo the dogs actions and well-being and shall hold NC Saint Bernard Rescue and its volunteers free from all claims for personal injury, injury to others, and/or property damage associated with this adoption and rescue dog. I understand that NC Saint Bernard Rescue has limited knowledge of this dogs health.

Adopter(s) agrees to pay reasonable attorney's fees and court costs in the event this matter is forwarded to an attorney for enforcement.

Adopter(s) agree to pay the sum of $500.00 as liquidated damages in the event the terms of this contract are breached.

I understand that the NC Saint Bernard Rescue will contact me from time to time throughout the dog's lifetime and agree to respond readily to inquiries about the rescue dog. They may also visit the dog at anytime with 48 hours prior notice.

I have read every provision above, including the liquidated damages clause, I agree to abide by every provision in this contract.
